November 2, 2026 — Ibogaine Club, an independent information and community platform, has launched its public Ibogaine Fatality Tracker to make information about reported deaths involving ibogaine and iboga easier to find, examine and update.
The tracker currently documents 12 individual cases across nine countries. Each record brings together available sources, reported circumstances and investigation outcomes, with separate fields for verification status and whether a causal role for ibogaine has been established. The database is freely accessible at ibogaine.club/fatalities.
Information about these deaths is scattered across news archives, court records, official statements and accounts from families. Some cases received substantial coverage when they occurred but little follow-up. Others remain poorly documented, leaving basic questions unanswered years later.
Ibogaine Club created the tracker to help close those gaps. Its aim is to support a more transparent, safety-conscious ibogaine space by making the evidence accessible and showing readers where that evidence ends.
Following Up When the Public Record Stops
The project includes original follow-up work alongside its aggregation of published sources.
In the case of Federico Fiuzzi, who died in Maribor, Slovenia, in 2011, Ibogaine Club contacted the District Court in Maribor to clarify a procedural history that had remained unclear in earlier coverage. The court explained that a prosecutor’s 2013 request for a judicial investigation was later withdrawn following the death of the person concerned, and that criminal proceedings were never initiated. The tracker distinguishes that procedural clarification from a finding about the medical cause of death or criminal responsibility.
Ibogaine Club has also received additional firsthand information from Reannon Allison, the sister of Marisa Davis, whose 2012 death in Utah is documented in the tracker. With her sister’s permission, the platform has expanded Marisa’s record while identifying details that have not been independently verified. Work to clarify the circumstances continues.
Documenting Cases Without Overstating the Evidence
A death following ibogaine exposure does not automatically establish that ibogaine caused it. Likewise, naming a treatment center in a case record does not, by itself, establish negligence or wrongdoing.
The tracker separates reported accounts from official findings and identifies uncertainty about causation. Sources may include news reporting, medical and legal records, provider statements and attributed family testimony. Readers can follow the cited material rather than rely on a summary alone.
The database is a developing record, not an exhaustive count of deaths worldwide. Its case totals cannot be used to calculate a fatality rate: reporting is incomplete, and the total number of people exposed is not reliably known. The tracker also covers different settings, including clinical treatment, private administration and traditional ceremonies, which should not be treated as interchangeable.
For Ibogaine Club, transparency means taking both potential benefits and serious risks seriously. The tracker is intended to support informed scrutiny, better reporting and a more accountable discussion of safety, without reducing the people behind the records to statistics.
An Invitation to Contribute
Ibogaine Club welcomes corrections, additional documentation and firsthand information from families, clinicians, researchers, journalists and treatment providers. Contributors are asked to distinguish what they witnessed directly from what they learned through others and to contact the team privately before sharing sensitive records.
